Southern Colorado home-based primary care team sees patients in their homes. “We see them so often they don’t have to go to the hospital or the emergency room. We’re able to keep them in their home.”
The Plano VA Outpatient Clinic opened in 2016 in a 10,000 square-foot space. The clinic has now added 10,000 square-feet of clinical space and added a seventh Patient Aligned Care Team.
VA’s patient care model puts a special emphasis on preventive care, and providers are given the tools and support to focus on preventing disease.
